A 15-year-old boy with a past medical history of atopic eczema is referred to Dermatology by his local emergency department having presented with a 4-day history of an itchy rash on his elbows and knees (see image below).

Observation of rash.

Over the last year he has also been complaining of lower abdominal pain, particularly after eating, feeling tired all the time and his friends have noticed he is often pale.
