A 20-year-old man underwent genetic screening aged 12 as 4 family members across 2 generations had previously been diagnosed with colorectal cancer. A germline APC gene mutation is found. He has undergone colonoscopy screening every year since his diagnosis. His most recent colonoscopy is reported to show >100 adenomatous polyps in the distal colon only without macroscopic evidence of malignant transformation. The largest polyp was 20mm in diameter. He is reviewed in the gastroenterology outpatient clinic to discuss management options.
