A 37 year old man is referred to the rapid access ENT clinic due to a 2 week history of nasal congestion associated with discharge, periodic epistaxis, and a change in voice. He is a non-smoker and is usually fit and well. The only past medical history of note is that an ulcer that appeared when he had temporarily moved to Brazil for work 4 years ago; it healed by itself, and so he did not seek medical advice at the time. Examination reveals an inflamed and erythematous nasal cavity, with erosions in the septum.
