A 44-year-old man with a history of alcohol dependence presents to A&E with severe abdominal pain and vomiting, which has worsened over the past 48 hours. On examination, he is clearly in distress and struggles to lie still for the examination. His abdomen is tender, and an ecchymosis is noted (see below).
