A 54-year-old male presents to the emergency department with right-sided leg weakness.

A 54-year-old male presents to the emergency department with right-sided leg weakness. His wife has noticed that he has been generally more confused and drowsy. He has a headache that was worse during the morning and on bending forward. On examination, he was noted to have a power of 3/5 throughout his right leg with a power of 4+/5 on his left leg. The tone was normal. His heart rate and BP were normal. A CT scan confirms the presence of a tumour with a mass effect. The cingulate gyrus has herniated through a dural structure.
