A 55-year-old man has noticed some painful sensation in his legs at night, reduced sensation in his feet, urinary frequency, and balanitis. He has a small area of painless ulceration on his big toe. He has a raised BMI. Nerve conduction studies show reduced motor conduction velocity, and EMG shows positive sharp waves and fibrillations.
