A 56-year-old male presents to the acute medical unit with confusion. Collateral history from the neighbour states that the patient was found outside his home wandering the streets. He appeared unable to recall his neighbours’ and family members’ names. He was found to be confabulating when questioned. His neighbour notes that he has been drinking approximately three bottles of wine daily for the past 20 years. Neurological examination was intact. An MRI of his brain reveals global cerebral atrophy with increased T2 signal within the mammillary body, thalamus and periaqueductal regions.
