She has a past medical history of NYHA II heart failure and a mechanical aortic valve replacement. She also takes losartan, bisoprolol, spironolactone and warfarin. She has no drug allergies. Upper GI endoscopy reveals a duodenal ulcer. The endoscopist performs a rapid urease (CLO) test, which is positive. She is treated with endoscopic clips and adrenaline injection.
