A 60-year-old female with a history of alcohol excess, coronary artery disease, and hypercholesterolemia

A 60-year-old female with a history of alcohol excess, coronary artery disease, and hypercholesterolemia, and a BMI of 32, presents to the emergency department with persistent diarrhoea and weight loss over the past four months. She describes her stools as frothy. On examination, she has mild to moderate epigastric pain without guarding or rebound tenderness. A rectal examination is negative for melena, and she denies experiencing reflux or postprandial pain. She is referred to the medical team, and an SHO orders an HbA1c test, which returns a result of 55 mmol/mol. A review of her GP records reveals that her HbA1c level was 45 mmol/mol twelve months ago. Further bloods tests show the following:

Sodium 134 mmol/l (135-145)
Potassium 3.3 mmol/l (3.5-5.0)
Adjusted calcium 2.1 mmol/l (2.1-2.6)
CRP 30 mg/l (<10)
Vitamin D 20 nmol/l (>50)
