A 65-year-old female presents with an eight-month history of weakness

A 65-year-old female presents with an eight-month history of weakness. She is unable to stand from a chair independently. She struggles with combing her hair and lifting objects above her head. She also had noticed increased difficulty swallowing over the past three months. Examination reveals weakness with power 3/5 in her shoulders and thighs and 4/5 power elsewhere. Sensory examination was normal. She had lilac discolouration of her eyelids and hyperpigmented plaques overlying her knuckles and finger joints. Red plaques were seen over her knees and elbows. Creatinine kinase levels were significantly raised.
