A 65-year-old lady with breast cancer with liver metastasis presents to clinic. She has been treated for abdominal pain

A 65-year-old lady with breast cancer with liver metastasis presents to clinic. She has been treated for abdominal pain previously with opioids to good effect. She takes 30mg oxycodone modified release twice a day, with 5mg of immediate release as required. Over the last few weeks, she has needed to use her immediate-release analgesic more often and is now taking this 4 times a day. This takes the edge off of her pain but does not take it away completely as it used to.

What would be the next step in her pain management?
