A 65-year-old man with a background of type 2 diabetes mellitus is brought to A&E with severe generalised abdominal pain and bloody diarrhoea. His observations are: BP 80/60 mmHg, RR 21/min, HR 109 bpm, T 38.8ᵒC, SpO2 100% on air. The physical examination revealed a tense abdomen and guarding in the LIF.
