A couple present with identical symptoms of fever, right upper quadrant pain and jaundice. They recently returned from a holiday in Peru, where they rented a small cottage near a farm and regularly used the nearby growing water chestnuts and parsley in their cooking. Examination reveals hepatomegaly.
