A medical student presents with a 4 day history of non-bloody diarrhoea, non-specific abdominal pain and itching. The itching preceded the abdominal pain and diarrhoea by approximately 2 weeks and had started a day before his return from Vietnam, where he spent time doing his elective. The itching is mainly on the soles of his feet, and he has noticed some red spots with associated surrounding swelling. On examination, his abdomen is soft with no tenderness or guarding, and you notice a papulovesicular rash on the soles of both feet, with signs of excoriation. He is haemodynamically stable, and blood tests reveal eosinophilia.
