An 18-year-old male presented to the cardiology unit following an abnormal ECG

An 18-year-old male presented to the cardiology unit following an abnormal ECG. His mother notes that he has had difficulty walking since infancy with delayed motor milestones during infancy. He had frequent falls, with reduced exercise tolerance during childhood. She describes that he used his arms to support himself on his thighs when standing from a lying position. He is currently a wheelchair user due to weakness in his limbs.

Examination reveals enlarged calf muscles, and a repeat ECG shows sinus tachycardia with right axis deviation and Q waves in leads I, aVL, V5 and V6. Serum CK was significantly raised at 20 times the upper limit of normal.
