An 18-year-old Turkish male presents to the acute medical unit with fevers

An 18-year-old Turkish male presents to the acute medical unit with fevers and generalised abdominal pains lasting approximately 24-48 hours before settling spontaneously. He had previous episodes of painful joint swelling affecting his knees and ankles. Examination revealed generalised guarding throughout the abdomen, but a CTAP only showed dilated mesenteric vessels. Blood tests revealed a mildly raised WCC and CRP. Autoimmune studies were negative, but ESR was raised at 30mm/hr. His father experienced similar symptoms without a formal diagnosis for these presentations.
