An 8-year-old boy is brought into A&E after presenting with fever, dry cough, wheezing, abdominal discomfort and reduced activity. The family are on holiday in Portugal and have only been there a week. They have been concerned because they have seen their son play in sandboxes that were being used by dogs to defecate in. There is no history of asthma or atopy in the family, and prior to this, the son has never been ill. Examination reveals hepatomegaly. Blood tests show a raised eosinophil count and elevated liver transaminases.
