An 86-year-old woman undergoes a colonoscopy following a 6-month history of loose stools and abdominal discomfort

An 86-year-old woman undergoes a colonoscopy following a 6-month history of loose stools and abdominal discomfort. She has a history of hypertension, chronic kidney disease (CKD), type 2 diabetes mellitus, severe knee osteoarthritis and mild cognitive impairment. She is found to have a tumour in the terminal ileum. Biopsies taken during the procedure confirm a gastrointestinal neuroendocrine neoplasm. A subsequent staging CT scan shows metastatic deposits in the liver. Her case is discussed in the GI neuroendocrine MDT, where it is felt that she would not be a suitable candidate for surgical intervention, and medical therapy is advised.
