You are a medical SHO in the acute medical unit.

You are a medical SHO in the acute medical unit. You see a 21-year-old man who was referred by his GP with recurrent epistaxis and easy bruising. The man seems awkward and has poor eye contact, but does tell you he has seen a dentist recently who was concerned about his teeth. He also complains of pains in his knees. He also reports feeling frequently sad, tired and that he struggles with anxiety but continues with his university studies. To manage, he has been smoking more and drinking more alcohol. On direct questioning, he reports not liking “wet” food, and diet often consists of dry cereal, ham sandwiches and chips. He denies any intention to lose weight.
