You are asked to review a 56-year-old woman on the ophthalmology ward. She presented to A&E 2 days ago with fevers and an acutely painful red left eye. On examination of the left eye, there is photophobia, lacrimation, and a small, fixed pupil. Her vision is blurred in that eye and hypopyon can be seen. She is also noted to have bilaterally enlarged parotid glands. Other than a recent right-sided facial nerve palsy, the woman has no past medical history. A biopsy of the swollen parotid gland via fine-needle aspiration reveals non-caseating granuloma.
