You review a 47-year-old man in A&E, who has a background of alcohol excess and a 30-pack smoking history. He reports a 2-week history of dull epigastric pain, nausea, bloating and reflux. He finds his symptoms are exacerbated a few hours after food. Examination is unremarkable, and observations within normal ranges. Blood tests reveal a mild microcytic anaemia but nothing else of significance.
