A 22-year-old laboratory assistant is seen in the eating disorders clinic after being referred by her GP. She has a BMI of 16.5 and has been using diet pills and thyroxine she bought over the internet to help her lose weight. Roughly once per week she will eat a box of doughnuts and feel so guilty afterwards that she makes herself vomit. She has frequent obsessive thoughts regarding eating too much or putting on weight.

She is reviewed by a medical student who is not sure of the diagnosis and asks the consultant for an explanation.
