A 22-year-old man presents to A&E, referred in by his GP due to a blood test abnormality

A 22-year-old man presents to A&E, referred in by his GP due to a blood test abnormality. He recently sent off a private blood test to check his health, which reported an elevated ALT. His GP repeated the test with a full panel of LFTs and added a CK level in addition. The results of these are below and prompted the GP to refer him urgently into A&E:

ALT 120 [10 – 35]

AST 136 [0 – 31]

Bili 10 [0 – 21]

ALP 58 [35 – 104]

GGT 37 [6 – 42]

CK 15,065 [20 – 200]

Repeat blood tests in A&E yield similar results. The man has no current symptoms. He has had no previous health problems, has no relevant family history and does not smoke. He drinks alcohol socially and admits to the occasional binge when out with friends. He is a keen gym goer but has been worried of late that he takes a very long time to warm up, although after 10 minutes or so he gets another burst of energy which sees him through. What is the most likely explanation?
