A 24-year-old gentleman originally from Sweden is referred for review by the medical team during an admission under the surgeons for recurrent episodes of biliary colic and cholecystitis. He was anaemic on admission and has evidence of mild splenomegaly. He has no known past medical history, and whilst his mother has no medical problems, he is lacking details about his father as they are not in contact, but knows that he does have some sort of blood condition and has needed blood transfusions in the past. Blood tests show:
| Hb (130 – 180 g/L) | 102 g/L |
| WCC (3.6 – 11.0 x 10⁹/L) | 10.5 x10⁹/L |
| Platelets (140 – 400 x10⁹/L) | 350 x10⁹/L |
| Bilirubin (<21 μmol/L) | 29 µmol/L |
| ALT (<41 U/L) | 24 U/L |
| ALP (30–130 U/L) | 64 U/L |
