A 24-year-old man presents to A&E with visual disturbance.

A 24-year-old man presents to A&E with visual disturbance. He reports that he woke up in the morning with left-sided visual loss and slurred speech which was noticed by his family after being well when he went to bed. He has a background of sickle cell disease and is known to be intermittently compliant with medications. On examination, he has a left homonymous quadrantanopia and is dysarthric. His oxygen saturations ate 97% on air. He has a CT head and a CT angiogram which do not demonstrate any haemorrhage. Haemoglobin is 68 g/L (130-180 g/L) and blood sugar level is within normal range.
