A 24-year-old man presents with bloody diarrhoea. He thinks this is due to a takeaway he had recently. He has not been abroad and he has no other medical problems.

Blood tests show: Hb 85 (120-150), Plt 72 (150-400), WCC 15 (4-11), Cr 170 (baseline 72), Ur 22 (baseline 8),  CRP 240 (<5).
