A 26-year-old female is referred to secondary care with a blood pressure of 220/140mmHg.  Fundoscopy showed generalised arteriolar narrowing bilaterally.  ECG shows large QRS complexes, particularly across the chest leads. Over the following days, she is reviewed by the endocrinology team who excluded most secondary causes of hypertension.  A magnetic resonance angiography of the renal arteries was requested.
