A 29-year-old gentleman presents with left leg swelling and pain. He reports worsening fatigue

A 29-year-old gentleman presents with left leg swelling and pain. He reports worsening fatigue over the last week or so and appears pale from the end of the bed with icteric sclera. Some mornings, he has noticed dark urine but thought it was because he was dehydrated. He reports that he was diagnosed with a right leg DVT around 6 months ago and has just finished a course of anticoagulation. Other than this, he has no past medical history and is currently not taking any regular medication.

On examination, his left calf is warm, swollen and tender to touch. His observations are in the normal range. Bloods are as follows:

Hb (130 – 180 g/L) 75 g/L
WCC (3.6 – 11.0 x 10⁹/L) 3.7 x10⁹/L
Neutrophils (1.8 – 7.5 x 10⁹/L) 1.4 x10⁹/L
Platelets (140 – 400 x10⁹/L) 57 x10⁹/L
Bilirubin (<21 μmol/L) 56 µmol/L
LDH (240 – 480 U/L) 1280 U/L
Haptoglobin (0.5 – 2.2 g/L) 0.1 g/L
D-dimer (< 500 ng/mL) 1200 ng/ml
Direct antiglobulin test No agglutination
