A 30-year-old woman presented with a 1-week history of feeling generally unwell and a three-day history of a palpable purpuric rash on her lower limbs. She had no past medical history of note and was prescribed no regular medication. Urinalysis showed blood +++ and protein +++. Investigations were as follows:
| Serum creatinine | 776 µmol/L (60–110) |
| Serum C-reactive protein | 226 mg/L (<10) |
| Urinary albumin : creatinine ratio | 136 mg/mmol (<30) |
| Urinary tract ultrasound | normal |
A renal biopsy was undertaken. The results demonstrate positive immunofluorescence for IgA, IgG and C3 but negative for other immunoglobulins and complements.
