A 32-year-old female with a background history of erratically controlled epilepsy is admitted with seizures. Her flatmate administered buccal midazolam after 5 minutes as per her usual emergency plan, terminating the seizure, but there was no emergence so he called for an ambulance. On their arrival, she was once more seizing, so paramedics gave rectal diazepam and transferred her to hospital under blue lights.
During handover, she is actively seizing. The attending paramedic reports that there was no emergence following rectal diazepam.
