A 35-year-old man presents to A&E complaining of severe lower back pain. He has a background of hypertension and renal transplantation for idiopathic end-stage renal failure and has been stable on immunosuppressive therapy for the past 6 years. He also recently underwent a steroid injection to his shoulder for bursitis. His back pain is described as sharp and has slowly developed to the worst pain he has ever experienced. He is noted to be febrile but is currently hemodynamically stable. On examination, you detect mild motor weakness and increased tone throughout his legs bilaterally. He describes no issues with his bladder or bowels.
