A 38-year-old never smoker presents with a 3-month history of fatigue, weight loss, fevers and a hoarse voice. On review of systems, he also reports pain in the left hand over this period. He has no significant past medical history. On examination, there is weakness of the left hand and a left partial ptosis. Chest X-ray reveals a left apical mass.
