A 45-year-old female attends a Neurology outpatient appointment for an annual review

A 45-year-old female attends a Neurology outpatient appointment for an annual review. She was first referred to a neurologist by her endocrinologist aged 41 with painful muscle spasms and difficulty walking. She reported the spasms seemed to mostly be triggered by a sudden noise or if someone lightly touched her and were worse when it was cold outside. She has a background of pernicious anaemia.

On initial assessment she was found to have widespread hypertonia, especially in muscles of the trunk, a subtly wide-based gait, easily elicited reflexes, normal power and normal sensation. On review today her symptoms are well controlled using diazepam, baclofen and gabapentin.

Which serum antibody is most likely to be positive in relation to this lady’s neurological diagnosis?
