A 49-year-old female patient presents to the Emergency Department with seizures. She has a background history of a failed renal transplant

A 49-year-old female patient presents to the Emergency Department with seizures. She has a background history of a failed renal transplant, for which she continues to take tacrolimus. Her end-stage renal failure is secondary to focal segmental glomerulosclerosis. Her last session of haemodialysis was the day before admission, and the dialysis nurses have recorded a complaint of persistent headache in the notes. Her blood pressure was also noted to be elevated.

On assessment in resus, she is having seizures, which terminate with administration of IV lorazepam. Her blood pressure is 184/99 mmHg. Post-ictally, she moves all of her limbs with equal power. On fundoscopic examination, no hypertensive changes are noted.
