A 53-year-old presents in extremis with severe diarrhoea, haemoptysis, and rigors. A gram-negative bacteraemia is noted from blood cultures. The patient’s medical history is scarce, except for a recent diagnosis of temporal arteritis. On examination, there is an urticarial rash on the patient’s back. They tell you this comes and goes and can be very itchy.
