A 58-year-old man is admitted having experienced seizures whilst out for dinner with his daughter

A 58-year-old man is admitted having experienced seizures whilst out for dinner with his daughter. He is visiting her from Mexico and was intending to return in the next few days.

His seizure self terminated after 3 minutes, and he arrived to the Emergency Department by ambulance several hours ago. He is now orientated but fatigued.

He tells you, via his daughter, that he is a farmer and has an excellent functional baseline. On systems enquiry, he reports recent headaches but had thought nothing of them.

Observations are within normal limits. Neurological examination, including fundoscopy, is unremarkable.

A full blood count is returned:

Haemoglobin 152 (130-180)
Platelets 329 (140-400)
White cell count 7.0 (3.6-11)
Neutrophils 3.1 (1.8-7.5)
Lymphocytes 1.5 (1-4)
Monocytes 0.3 (0.2-0.8)
Eosinophils 2.1 (0.1-0.4)

An unenhanced CT brain demonstrates multiple foci of calcification within the brain parenchyma.
