A 68-year-old man was seen on the acute stroke unit. He has been receiving therapy

A 68-year-old man was seen on the acute stroke unit. He has been receiving therapy delivered by the multidisciplinary team for his left MCA stroke which had resulted in significant hemiparesis and dysphasia. The physiotherapist asks you to review the patient because he had been working well with them but today has struggled with their session and declined to finish the session as he appeared to be in pain. On examination, he has weakness of the right upper and lower limbs, his arm is by his side in the bed. There is reduced tone of the right arm. You note that there is some asymmetry between the left and right shoulder and you palpate a one-finger gap below the left acromion. What is the most likely diagnosis?
