A 68-year-old woman is referred to the neurology clinic. She initially presented to A&E with lower back and hip pain following a fall. She was found to have a fractured left hip and went on to have a hemiarthroplasty. She has been referred because, despite being very motivated to improve, she is failing to make progress with the physiotherapists.
On examination, hip flexion seems to be good given her recent surgery, however, she has marked weakness of the quadriceps on the left and numbness over much of the anterior thigh and into her medial knee and shin. Patellar reflex is absent on the left but normal on the right side.
