A 72-year-old female is brought into hospital by her family. She had a stroke a year ago

A 72-year-old female is brought into hospital by her family. She had a stroke a year ago and, since that time, has found it difficult to concentrate on tasks. For the past 72 hours, her family have reported difficulty in recognising family members at times, including her husband.  She has no motor or sensory deficits on examination. Her Blood pressure is 163/95 mmHg. She takes a Statin, Clopidogrel and Ramipril. CT imaging of her head shows no evidence of acute infarct. An MRI of her head shows widespread white matter ischemic changes. Her Prosopagnosia shows no signs of improvement. Given the most likely diagnosis, what is the most appropriate management plan?
