A 72-year-old man with metastatic squamous cell carcinoma of the lung is seen in the oncology clinic for a routine review of his palliative chemotherapy. Despite his treatment, he is suffering from worsening exertional breathlessness. Although he has extensive metastatic disease, he has never suffered particularly with other symptoms such as cough or pain. A repeat CT chest suggests that the dyspnoea is related to progressive disease burden.

Which medication could his oncologist offer for symptomatic relief of his breathlessness?
