A 73-year-old man is referred to the respiratory clinic due to increasing shortness of breath and cough for the last year. His past medical history includes ischaemic heart disease, atrial fibrillation, non-Hodgkin’s lymphoma and osteoarthritis. He has forgotten to bring his medication list with him. He is allergic to penicillin.

His high-resolution CT scan shows triangular-shaped, patchy ground glass opacities peripherally with some air bronchograms.
