A 76-year-old man presents to the respiratory clinic with poorly controlled COPD. Despite taking his Symbicort (budesonide/formoterol) and salbutamol inhalers regularly, he has noticed increasing shortness of breath and now finds himself breathless on walking more than a few metres. He is comfortable at rest and observations measured in clinic are within normal limits. What would the next step in his management be?
