A 77-year-old female is admitted following a complicated neck of femur fracture

A 77-year-old female is admitted following a complicated neck of femur fracture after a fall at home. She undergoes a hip hemiarthroplasty which is successful and is prescribed 1 unit of RBC post operatively.

You are called to review her overnight, as 30 minutes into the transfusion she complains of chills, back pain and nausea. Her observations are T 38.5℃, sats 96% on room air, RR 24/minute, HR 110 bpm, BP 90/42 mmHg. You notice some oozing from her cannula site and some new rose-coloured urine in her catheter tubing. You immediately stop the transfusion and give IV NaCl 0.9% and send the blood back to the lab for analysis.
