Mr Henry is a 65-year-old man who is currently being managed for a severe diabetic foot infection. Recent wound cultures showed resistant staphylococcus aureus which was sensitive to vancomycin. Microbiology suggested starting vancomycin and aiming for higher target serum levels. He was given a loading dose of 1500mg followed by a maintenance dose of 750mg once daily, these doses are appropriate for his weight and renal function. A trough level was taken just before the 4th dose which came back as 12mg/L. How should this patient’s vancomycin therapy be changed given the serum vancomycin level?
