You are asked to see an 80-year old man on the stroke ward

You are asked to see an 80-year old man on the stroke ward out of hours on a Sunday evening. He was admitted 2 days ago with an MCA infarct and has been on NG feed since then. He is known to be a type 2 diabetic and is on Metformin 1g BD.  His blood sugars have been 12 – 14 mmol/L over the last 48 hours and the nursing staff are concerned about his hyperglycaemia.
