Your next patient of the day is James, a 4-year-old boy who tiptoe walks into your consultation room with his father. His father says James has just started in reception, and it has been challenging to get him there in the first place as he often becomes very distressed. Once there, he also seems very obsessed with organising all the toys. When you ask James about why he does this, he doesn’t look you in the eyes but replies, ‘What a ridiculous question! It’s my imperative!’
