A 12-year-old boy is brought to A&E by his parents, after waking during the night with severe pain in his left scrotum and lower abdomen. He is tachycardic and hypertensive. On examination, the left testicle is high riding, swollen and appears blue under the skin. It is tender, but the pain can be slightly eased by scrotal elevation, and the cremasteric reflex is absent.
