A 12-year-old boy presents to the GP with a 3-day history of moderate pain in his right scrotum. The pain gradually increased over the first day and has remained the same since. He is otherwise well. On examination, the scrotum appears normal; however, there is a nodule palpable at the upper pole. There is focal tenderness over the nodule, and when the skin is stretched over the nodule it appears blue. The cremasteric reflex is present.
