A 13-year-old boy is brought to his GP by his mother. She is very worried as he has lost a lot of weight

A 13-year-old boy is brought to his GP by his mother. She is very worried as he has lost a lot of weight during the past six weeks. He had been living with his father, whom she is separated from, whilst a custody dispute was resolved. She has noticed that since she has started having him at home again, he has become an increasingly fussy eater, turning down all of her meals. He tells you that he “hates being fat” despite being visibly underweight and that people at school have teased him about his weight.
