A 15-year-old girl attends the GP with her mother. She has been having episodes of recurrent vomiting

A 15-year-old girl attends the GP with her mother. She has been having episodes of recurrent vomiting for around six months, but despite this her mother reports she is eating well. Her BMI is measured at 22 and she has not recently lost any weight according to both the patient and her mother. The patient tells you she is not worried, as she thinks she is healthy, and is only at the doctors because her mum has forced her. On examination, significant dental erosion is noted and the patient has several calluses on her knuckles.
